LPN Geriatric Behavioral Health - FT - All Shifts

Job Summary Join our dedicated healthcare team as a Full-Time Licensed Practical Nurse (LPN) specializing in Geriatric Behavioral Health, working across all shifts to provide compassionate, comprehensive care to our senior residents. In this vital role, you will deliver high-quality nursing services tailored to individuals with complex behavioral health needs, including dementia and Alzheimer's care. Your expertise will help promote safety, stability, and well-being for our elderly population in a supportive environment. Responsibilities Conduct thorough patient assessments, including vital signs, physical examinations, and behavioral observations to inform individualized care plans. Administer medications and treatments accurately, including injections, IV infusions, tube feedings, and catheter care, adhering to all safety protocols and HIPAA regulations. Monitor patients’ mental health status and behavioral changes, implementing de-escalation techniques and collaborating with interdisciplinary teams to optimize outcomes. Manage medical documentation efficiently using electronic health record (EHR) systems such as Carevue/Medhost, ensuring compliance with healthcare standards. Assist with diagnostic evaluations and coordinate care activities such as laboratory specimen collection, specimen processing, and medical imaging procedures. Provide education and support to residents and their families regarding disease management, medication administration, and discharge planning. Maintain infection control practices and aseptic techniques during all patient interactions to ensure a safe environment for residents and staff. Qualifications Valid LPN license with current state licensure required. Experience working in geriatric settings such as assisted living facilities, nursing homes, or long-term care environments preferred. Critical care experience or hospital medicine background highly desirable; Knowledge of dementia care, Alzheimer’s disease management, memory care strategies, and behavioral health interventions essential. Proficiency with EMR/EHR systems such as Carevue/Medhost; Strong understanding of medication administration protocols including injections, IV infusion techniques. Excellent patient assessment skills coupled with physiology knowledge; ability to work effectively with individuals with developmental disabilities or mental health challenges. Basic life support (BLS) certification required; additional certifications such as IV insertion or phlebotomy are beneficial.
