Please let me know if you or any of your friends would be interested in this position.Position Details:Social Worker - Clinton, TNLocation : Clinton, Tennessee 37716Project Duration : 12 Months +Salary Range : $20/hr to $22/hrShift : 8:00am to 4:30pmJob SummaryThe Social Worker supports primary care providers and patients through case management, social support navigation, referral coordination, patient education, and community resource collaboration to improve continuity of care and patient outcomes.Key ResponsibilitiesSocial Support Navigation / SDOHAssist patients with social determinants of health (SDOH) needs including food insecurity, housing insecurity, and access to community resourcesCompile and maintain resource lists including eligibility criteria, referral processes, and contact informationConduct in-person or remote social needs screenings/assessmentsCollaborate with primary care nurses, providers, and social workersCoordinate or connect patients with social services resources including housing, food, clothing, and mental health servicesCase ManagementSupport discharge planning, follow-up, and patient education with hospitalsAssist with obtaining patient records from hospitals and specialistsHelp secure medical equipment through community partnersConduct follow-up on patient care plansIdentify patients overdue or lost to follow-up and assist them in returning to careReferral & Prescription AssistanceSchedule, coordinate, and track specialist and imaging referralsMaintain referral resource lists including eligibility, cost, and contact informationAssist patients with low-cost prescription resources and patient assistance programsSupport applications for programs such as Cover Rx and Rx OutreachDocumentation & Patient SupportDocument in patient records and update consults as neededTag providers and clinical staff appropriately within recordsProvide patient education and identify appropriate educational resourcesAdditional ExpectationsComplete onboarding and orientation requirementsParticipate in clinical, provider, and regional office meetings as requestedAttend Health Councils and community meetings to build relationships with social service agenciesIdentify barriers to patient care and assist in developing solutionsTo discuss this amazing opportunity, reach out to our Talent Acquisition Specialist Nikhil Sai at email address can be reached on # (630) 576-1935 .About generis tek : generis tek is a boutique it/professional staffing based in Chicago land.
