Job Description Seventh Haven Care Services is seeking a compassionate and skilled Registered Nurse (RN) or Licensed Practical Nurse (LPN) to provide comprehensive clinical care to clients in a home care setting. This role includes conducting health assessments, assisting with care planning, and performing skilled nursing procedures to ensure each client's medical needs are safely and effectively managed in their home.Conduct initial and ongoing client health assessmentsAssist in the development and implementation of individualized care plansProvide skilled nursing services, including medication administration, wound care, and treatments as orderedMonitor client health status and communicate concerns to the care teamSupport and follow established care plans to ensure continuity of careMaintain accurate and timely clinical documentation in compliance with state and agency standardsEducate clients and caregivers on health conditions, medications, and care techniquesEnsure compliance with Minnesota DHS regulations, as applicableCollaborate with physicians, RNs, case managers, and other team membersQualificationsFor Registered Nurse (RN):Current, active RN license in MinnesotaAssociate or bachelor's degree in nursing (2–4-year degree required)For Licensed Practical Nurse (LPN):Current, active LPN license in MinnesotaCompletion of an accredited practical nursing programFor All Candidates:Strong clinical and communication skillsAbility to work independently and as part of a teamReliable transportation (if travel is required)Preferred QualificationsExperience with medication managementExperience with tracheostomy (trach) careExperience with wound care managementHome care or community-based experienceTraining will be provided for the right candidate.
