This is an active, community-based position—not a traditional desk-centered case management role. You will work directly with a small caseload of adult members, helping them strengthen the skills, confidence, and connections needed to live as independently as possible. Your responsibilities will include: * Developing positive, respectful, and professional relationships with the members you support * Teaching independent living skills, including household maintenance, organization, budgeting, and time management * Assisting with meal planning, grocery shopping, and food preparation * Supporting personal hygiene, healthy routines, and other daily living needs * Monitoring and supporting medication routines when required * Helping members strengthen social skills and build meaningful community connections * Promoting personal wellness, home safety, self-advocacy, and informed decision-making * Connecting members with community resources and opportunities * Maintaining accurate documentation and communicating important information to the appropriate team members * Collaborating with coworkers, families, service coordinators, and other members of each person’s support team * Following through on assigned responsibilities and responding proactively when members need assistance *Who Will Thrive in This Role* You may be an excellent fit if you: * Approach your work with patience, optimism, and respect * Are dependable and understand that consistency matters to the people we serve * Enjoy helping others reach personal goals and become more independent * Communicate clearly, professionally, and compassionately * Take initiative and use sound judgment when working independently * Value collaboration and contribute positively to a team * Are receptive to feedback and interested in learning and professional growth * Can balance compassion with appropriate professional boundaries *Qualifications* * Experience supporting individuals with intellectual or developmental disabilities, autism, mental health needs, or learning disabilities is preferred * Strong communication, organization, and interpersonal skills * Ability to work independently while remaining connected and accountable to a collaborative team * Availability to work at least one weekend day each week * Valid California driver’s license and a clean driving record * Ability to pass a Department of Justice background check * Ability to complete a physical examination and TB testing * Adult CPR and First Aid certification, or willingness to obtain certification after hire *About Adaptive Learning Center* Founded in 1988, Adaptive Learning Center is a nonprofit organization based in Concord, California.
