As an NP or PA, you’ll complete annual in-home health assessments for Medicare Advantage members.This is a non-prescribing, field-based role focused on prevention, patient education, and closing care gaps in partnership with PCPs.What You’ll DoPerform in-home health assessments (history, meds, vitals, exam)Complete screenings and point-of-care testing as neededIdentify and document conditions for care planningShare findings with PCPs to support follow-up careEducate patients on conditions and medicationsAddress social needs and connect patients to resourcesRecognize urgent issues and escalate when appropriateRequiredNP: ANCC or AANP certification (Family, Adult, Geri, or AGPCNP)PA: NCCPA certification + state licenseActive state license (or ability to obtain)Valid driver’s license + reliable transportationAbility to perform in-home visits (mobility + lifting equipment)Preferred1+ year clinical experience (primary care, geriatrics, or home health)Comfort with EMR/technologyExperience working with Medicare/geriatric patientsAt UnitedHealth Group, our mission is to help people live healthier lives and make the health system work better for everyone.
