Role Description We are seeking a Primary Care Nurse Practitioner (NP) or Physician Assistant (PA) to provide comprehensive care to patients within skilled nursing facilities and assisted living communities. This is a full-time hybrid role based in Melbourne, FL, with responsibilities split between in-field patient visits and remote administrative work. Key responsibilities include performing thorough assessments, diagnosing and managing acute and chronic illnesses, developing care plans, coordinating with interdisciplinary teams, and providing education to patients and their families. This role plays a vital part in transitional care and improving patient outcomes. Qualifications Valid Nurse Practitioner or Physician Assistant license in FloridaClinical expertise in diagnosing and managing acute and chronic conditionsProficiency in patient assessments and development of care plansStrong communication and interpersonal skills to collaborate with interdisciplinary teams and educate patients and familiesExperience in post-acute care settings such as skilled nursing facilities, assisted living communities, or rehabilitation centers is a plusAbility to work independently in a hybrid environment (in-person and remote)A commitment to providing value-based care and improving patient outcomesCurrent BLS/ACLS certification
