Develops individualized plans of care in collaboration with the patient, family, and healthcare providers, ensuring continuity and documentation. Implements interventions from the plan of care safely, timely, and appropriately, documenting actions taken. Evaluates patient progress towards outcomes, revising diagnoses, outcomes, and plans of care as needed. Administers medications and treatments as prescribed, ensuring patient safety and adherence to protocols.
