Practice Clinical Lead
We are seeking an experienced and driven Nurse Practitioner (NP) to serve as our Practice Clinical Lead in an on-site leadership role based in Orangeburg, NY.This position combines clinical care, team leadership, operational oversight, and program development. You will play a key role in managing the NP team, ensuring compliance, and driving performance across all clinical programs.Key ResponsibilitiesClinical Leadership & Oversight• Provide oversight across CCM, RPM, BHI, and telehealth programs• Supervise and mentor Nurse Practitioners and clinical staff• Ensure compliance with CMS guidelines, HIPAA regulations, and internal standardsTraining & Development• Train and onboard new Nurse Practitioners• Provide ongoing coaching and performance management• Foster a high-performance, accountable clinical teamQuality Assurance & Compliance• Conduct audits of documentation and clinical workflows• Ensure timely and accurate charting (within 48 hours)• Maintain audit readiness and regulatory complianceProgram Development & Operations• Develop and refine clinical templates and workflows• Collaborate with operations and field leadership• Support rollout of new programs and technologiesCare Coordination• Communicate care gaps and findings to partner facilities• Work with interdisciplinary teams (LPNs, care coordinators, facility leadership)• Oversee NP-to-facility assignments and team structureDirect Patient Care• Conduct approximately 50 virtual patient visits per week• Provide care across CCM, RPM, and BHI programs• Develop individualized care plansQualifications• Active Nurse Practitioner (NP) license• Willingness to obtain and maintain multi-state licensure (TX, FL, PA required)• Experience in CCM, RPM, telehealth, or value-based care preferred• Proven leadership or management experience• Strong communication, documentation, and clinical decision-making skills• Ability to work on-site in Orangeburg, NYCompensation & Schedule• Salary: $170,000 – $180,000 per year with performance-based bonus opportunities• Schedule:Monday–Thursday: 9:00 AM – 6:00 PMFriday: 9:00 AM – 1:00 PMBenefits• 15 days PTO annually (after onboarding period)• 5 paid sick days• Paid major U.S. federal holidays• Leadership growth opportunitiesWork Environment• In-office leadership role in Orangeburg, NY• Fast-paced, high-growth healthcare company• Direct collaboration with executive and operations teamsIdeal CandidateExperienced NP ready to lead and scale a clinical team Strong in both patient care and operational leadership Comfortable working in a structured, in-office environment Motivated to improve care quality and team performance#IND1
