Job TypeFull-timeDescriptionPOSITION PURPOSE: To provide services to in-home care clients in accordance with licensure rules, agency policy and procedures and client service planned needs and requests.ESSENTIAL DUTIES AND RESPONSIBILITIES Include The FollowingTo provide the safe provision or, or assistance with personal care tasks in accordance with OAR 333-536-0045: Bathing Personal grooming and hygiene Dressing Toileting and elimination Mobility and movement Nutrition/hydration and feedingTo provide the safe provision of, or assistance with medication services in accordance with OAR-536-0075: Assistance with self-administration of non-injectable medication Medication administration Medication management Medication administration through delegation in accordance with OSBN rulesTo provide the safe provision of, or assistance with housekeeping and supportive services including: Housekeeping tasks Laundry tasks Shopping and errands Transportation Arranging for medical appointments Meal planning and preparation Social contact and reassuranceTo provide client services in accordance with the service planTo help ensure the service plan is accurate and currentTo document services in accordance with agency policy and procedureTo ensure client rightsTo participate in all required initial and ongoing trainingTo report any accidents, injuries or suspicions of abuse or neglectMaintains the confidential of the work environmentIs familiar with the current condition of each clientOrganizes tasks efficiently to deliver client careRelate positively to supervisor, other employees, clients and their familiesOther duties as assignedRequirementsQUALIFICATION REQUIREMENTS:Ability to facilitate clients concerns using sensitivity and confidentiality on a daily basis.Ability to work independently with multiple tasks, interruptions, and prioritiesHas good communication skills both verbal and writtenHas ability to maintain good working relationship with all co-workers and the publicSuccessfully completes in-home care provider training programAbility to stoop, bend, squat and move a maximum of 40 pounds.Current valid driver’s license and proof of insurance coverageNON-ESSENTIAL JOB FUNCTIONS Include The FollowingAll aspects of the job are essential.Universal PrecautionsExposure to Blood/Body fluids is possible.Qualification RequirementsSuccessfully pass Clatsop Care Center Health District’s in-home care program trainingCurrent CPR and First Aid Training or ability to obtain CPR/First Aid cards with in 30 days of hire.PHYSICAL DEMANDS: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
