Registered Nurse (RN) | Case Manager Location: Orange, CA Agency: job.com LLC Pay: $2,900 to $3,000 per week Contract Duration: 13 Weeks Start Date: ASAP About the Position Position Overview We are seeking a dedicated and experienced Registered Nurse (RN) – Case Manager to provide comprehensive care coordination, particularly for patients with neuro-oncology needs. The RN Case Manager will play a key role in developing and managing individualized care plans, ensuring seamless coordination across services and providers. Key Responsibilities Develop and manage customized patient care roadmaps Coordinate follow-up appointments and ongoing care services Order and track labs, imaging, and diagnostic tests Facilitate referrals and obtain necessary authorizations Coordinate Durable Medical Equipment (DME) orders and delivery Provide end-to-end care coordination across multidisciplinary teams Lead transition planning (e.g., hospital to home or other care settings) Conduct patient needs assessments and adjust care plans accordingly Work closely with neuro-oncology patients and care teams Requirements Required Qualifications Active Registered Nurse (RN) license Experience in case management and care coordination Familiarity with neuro-oncology patient care Strong organizational and communication skills Certifications Required: PALS (Pediatric Advanced Life Support) or PEARS (Pediatric Emergency Assessment, Recognition and Stabilization) Preferred: CPHON (Certified Pediatric Hematology Oncology Nurse) How to Apply: Please contact us and share your updated resume: Ilma Shahbaz Phone: (512) 201-6582 Email:
