Resp & QualificationsAPPLICANT MUST RESIDE IN MONTGOMERY OR PRINCE GEORGE'S COUNTYPurposeThis position will support the Maryland government programs within the Health Services division, working as a bridge between the member, clinical and administrative staff. The Navigator helps members, families, and caregivers access the health care delivery system through outreach, education, follow-up, coordination support, and resources. Outreach includes both telephonic and community-based outreach, including home-visits with our members. We are looking for an experienced professional to work remotely within Montgomery & Prince Georges County, as well as in the surrounding region. While this position is remote, the incumbent will be expected to come into a CareFirst location periodically for meetings, training and/or other business-related activities, in addition to traveling throughout the community they serve to support our members.In addition, The Community Health Navigator participates in community-based events, representing the face of CareFirst and its efforts to promote the health and wellness of our member and their communities.Essential FunctionsAct as a liaison to ensure members are connected with appropriate care channels (i.e. pharmacy, insurance, billing, referral) and have the information they need to successfully navigate their care journeys.Outreach and follow-up with patients in between episodes of care as appropriate to promote a seamless and predictable healthcare experience. Ensure that member went to appointments and assist with coordination for transportation and pharmacy needs.Collect all information and relay to Care Coordination team. QualificationsEducation Level: High School Diploma or GEDExperience: 3 years' experience in a community relations, outreach, customer service or health educator roleLicenses/Certifications Upon Hire RequiredCHW Certification - Maryland Community Health Worker CertificationPreferred QualificationsCrisis Intervention skillsSkilled in typing and working within various web-based platformsExperience with Maryland Medicaid/Medicare recipient populationReliable transportation used for member engagementBilingual- fluent in SpanishAbility to have a flexible scheduleKnowledge, Skills And Abilities (KSAs)Reliable transportation to locations for member engagement.Strong written and verbal communication skills.Problem-solving skills to tackle various tasks and challenges/ability to have a flexible schedule.Skilled in typing and working within various web-based platforms.Crisis intervention skills.Must be able to meet established deadlines and handle multiple customer service demands from internal and external customers, within set expectations for service excellence.
