Job Title: Travel LPN / LVN - Long Term Care (LTC)Location: Glenwood, MinnesotaShift: Day Shift - 40 Hours/WeekAssignment Length: 9 WeeksWork Setting: OnsitePay Rate: $35/hour on W2Position SummaryWe are seeking a dependable and compassionate LPN/LVN for a 9-week assignment in a Long-Term Care (LTC) and Rehabilitation setting. The ideal candidate will have prior LTC experience and the ability to provide quality patient care in a fast-paced healthcare environment.Key ResponsibilitiesProvide direct nursing care to long-term care and rehabilitation patientsAdminister medications and treatments as prescribedMonitor and document vital signs and patient health statusPerform blood glucose monitoring and related patient careAssist patients with activities of daily living while ensuring safety and comfortMaintain accurate documentation within EMR systemsCollaborate with physicians, RNs, and interdisciplinary healthcare teamsFloat between facility units as neededFollow infection prevention and patient safety protocolsRespond appropriately to changes in patient conditionRequired QualificationsActive LPN/LVN license requiredMinimum 1 year of LTC experience requiredCurrent BLS Certification requiredExperience in Skilled Nursing Facility or Nursing Home settings preferredFamiliarity with EMR systems; Epic experience preferredSkills & CompetenciesStrong clinical and patient care skills in LTC settingsExperience with blood glucose testing and monitoringAbility to manage higher patient ratios effectivelyExcellent communication, teamwork, and documentation skillsFlexibility to float between units as neededFor more details reach at vgill@navitassols.comor Call / Text at 516-862-1203
