Program Coordinator

Divine House, Inc. is seeking a kind, compassionate and responsible Program Coordinator (PC) to supervise all aspects of the daily operation of a licensed group home site in Litchfield, MN. The qualified individual (PC) will have the ability to effectively supervise direct care staff to ensure the health, safety, and protection of each person served in the program. the PC will also be responsible for overseeing the implementation of each person's served Coordinated Service and Support Plan (plan of care). The PC will ensure that they and their staff are providing the necessary support each individual to ensure that their needs, interests, and preferences are respected and provided for in a safe and healthy environment that maximizes personal success. The PC must have the ability to communicate effectively, orally and in writing, and be able to create documents that are legible and accurate. Must pass Minnesota Department of Human Services background. Must have a valid drivers license.
