Come talk to us about joining the Centerstone team!JOB DESCRIPTION:SUMMARY OF POSITIONResponsible for development and implementation of individual care plans for low-risk clients and identifying the need for individual care and follow up with a Qualified Nutritionist.ESSENTIAL DUTIES & RESPONSIBILITIESComplete MIS application, certification procedure, and documentation including height, weight, and hemoglobin.Evaluate client's nutritional status through dietary assessment and consultants and make recommendations in relation to nutritional needs, family income, cultural food patterns, home facilities, and modifications in diet.Follow guidelines supported by a Participant Centered ApproachCreate and tailor a food prescription using the MIS application that is appropriate to the client's needs.Document assessments, evaluation, counseling, and referrals on the MIS application.Provide educational programs for clients in the form of classes or demonstrations and maintain related files and records.Provides all pregnant and breastfeeding women with nutrition education opportunities that promote and support breastfeeding.Work cooperatively with other staff and community agenciesParticipate in in-service education and staff meetingsAttend conferences and meetings needed or required by the positionMay supervise other staff.KNOWLEDGE, SKILLS & ABILITIESEffectively communicate via written, verbal, in person and virtual methods.Able to work independentlyFlexible during periods of program changePractices professionally in a manner consistent with legal and ethical standards including WIC regulations.Able to perform computer related tasks.Special literacy and language skills appropriate to address the culture of the population served by the local agency may be advantageous.QUALIFICATIONSEducation LevelBachelors degree in Dietetics; Nutrition or Nutrition Sciences, Public Health Nutrition; Community Nutrition or Clinical Nutrition required.Years of ExperienceN/ACertification/LicensureMust be either a Registered Nurse (R.N), Qualified Nutritionist, Registered Dietitian, Registration eligible to write the exam for the Commission on Dietetic Registration.PHYSICAL REQUIREMENTSTask / Percentage of TimeStanding - 20%Sitting - 80%Squatting - occasionalDriving - occasionalKneeling - occasionalLifting - occasionalBending - occasionalDISCLAIMERThe duties and responsibilities described in this document are not a comprehensive list and additional tasks may be assigned to the employee from time to time, or the scope of the job may change as necessitated by business demands.Time Type:Full timePay Range:$48,300.00--$67,700.00Delivering care that changes people's lives starts with our employees.
