We are seeking a dedicated Care Coordinator to join our team based in Casa Grande, Arizona. The ideal candidate will play a crucial role in assisting with client care and healthcare providers by coordinating care services and ensuring smooth day-to-day operations.

Duties
Complete Client Care Plan
Coordinate care with Case Managers
Complete monthly Home Monitoring visits
Assist with screening and onboarding of new caregivers
Other administrative tasks as assigned
Skills
Ability to manage and prioritize
Excellent Communication SKills
Have a good driving record and valid Driver's License
