POSITION SUMMARY: The Community Health Representative is responsible for providing assistance and the coordination and development of activities for elderly and physically or mentally disabled Tribal members. Performs community health duties to include patient transport and monitoring, home visits, conducting classes, and providing community education on Tribal health issues. Home Care may include house cleaning, personal hygiene, preparing meals, and other health and wellness related activities.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
1. Schedules and provides transportation for medical appointments and treatments and speaks for and on behalf of Tribal members to nurses, doctors, pharmacists, dentists, and other health care providers.
2. Makes regular home visits with homebound tribal members and assesses health, safety, and mobility issues, making referrals for additional services including, but not limited to, construction and remodeling assistance.
3. Provides health care services to include, monitoring blood pressure, vital signs, and other physical conditions, performing health assessments, while following medical instructions carefully and consistently.
4. Maintains confidential client files and prepares and presents reports and information as required for the community and home health service’s supervisor to manage and distribute resources effectively. Records services provided on timesheet and submits monthly reports.
5. Regularly inspects vehicles, performing safety checks and scheduling regular maintenance, cleaning, and repairs.
6. Assists in the development and coordination of health education events and activities.
7. Plans, prepares, and presents first aid, health, and disease information to the community.
8. Provides case management for clients through discussions with medical staff.
9. Provides emergency first aid and CPR when needed.
10. Works a flexible schedule as needed by the department.
11. Accompanies and assists patients in private homes, the community, and health facilities.
12. Performs on-call duties to include providing transportation to medical facilities and responds to non-emergency calls from clients regarding their health status.
13. Assists in the operation of programs within approved fiscal limits.
14. Monitors program participant progress using appropriate medical equipment and communication with the Southern Ute Health Center
15. Attends meetings and training when required or requested.
16. Participates in a positive, supportive, and team-oriented environment.
17. Performs other work-related duties as assigned.
QUALIFICATIONS:
Must have a High School Diploma or Equivalent.
Must have 6 months experience or training in a work setting in a medical field or working with and aiding the elderly and physically or mentally disabled.
Must have basic computer skills in software programs including Microsoft Word, Excel, and Outlook.
Must have experience providing personal care, i.e., toileting, bathing, grooming.
Must have or be able to obtain experience with medical equipment such as blood glucose monitor, blood pressure cuff/stethoscope, and pulse oximeter.
Must have or be able to obtain Certification in First Aid and CPR within 3 months of employment.
Must be able to work flex hours and maintain an on-call schedule.
Must have a valid Driver’s License for state of residency and be insurable under the Tribal vehicle insurance policy.
Must pass a thorough background check including Criminal History, fingerprint check, and pre-employment drug test.
PREFERRED QUALIFICATIONS:
One year’s experience in personal care, patient mobility and transfer, or the medical care.
Experience giving public presentations and instructing individuals in a class setting.
Ability to speak and write the Ute language.
C.N.A. (Certified Nurse’s Assistant)
CHR Basic 100 Certification.
