The Provider will function in collaboration with Physicians according to a Collaboration Arrangement. The Provider's practice will focus on caring for geriatric and seriously ill populations to assist patients and families with comfort and quality of life.In this role you will be responsible for: Conducting medical evaluations of predominantly elderly, frail, seriously ill patients in their home environment Determining follow-up assessments according to patient needs and best practice guidelines Communicating significant findings, problems, and changes in condition or environment to the clinical coordinator, the physician, family, facility, and other personnel involved with patient care Assessing health status by obtaining relevant health and medical historyPerforming a physical exam based on the age and patient history as per the Standard Care Arrangement with a collaborating physician Ordering, interpreting, and evaluating laboratory tests and other diagnostic studies to assess patient's clinical problems and healthcare needs Performing diagnostic or therapeutic interventions consistent with certification, law, and state medical privileges Prescribing and monitoring medications as allowed by prescriptive authority through the appropriate State Board Agency Developing a treatment plan in collaboration with the patient and caregivers Referring patients to physicians as appropriate for consultation or for specialized health resources and treatment Ensuring continuity of care by serving as a liaison between patient and other members of the multidisciplinary care team or with other specialty areas Providing patients and families an understanding of the diagnosis and disease progression while decreasing unnecessary emergency room and hospital visits Holding case conferences with families when appropriate to advance the care plan Meeting minimum productivity requirements based on practice standards. Required QualificationsMaster's or doctoral degree as a Board-Certified Nurse Practitioner or Physician AssistantCurrent State License as a Nurse Practitioner or Physician AssistantSit, stand, bend, lift, and move intermittently Must be able to lift at least 35lbs and bear the weight of an average adult effectively Current and valid driver's license Reliable transportation and valid automobile insurance coverage Demonstrates ability to assess and respond to the needs of patients, families, and caregivers in skilled nursing facility settings Demonstrates skill in practice, ability to cope with family emotional stress, and tolerance of individual lifestyles Ability to handle multiple priorities, documentation requirements, and deadlines Desired QualificationsPost-acute or Nurse Practitioner/Physician Assistant experience preferredAdult, Family, or Gerontology certification preferredExperience with geriatric patients preferredLocation Office Location: Kalamazoo, MIMain Service Area: Kalamazoo and St. Joseph or Hastings/Battle Creek and surrounding areasApproximately 1 day per week outside of the Kalamazoo area pending census This position services patients in facilities and private home settingsHours8:00 AM – 5:00 PM, Monday through Friday Pay Range$100,000—$120,000 USDBackground ScreeningOptimal Care conducts a background screening upon acceptance of a contingent job offer.
