LCSW – Clinical Social Worker | Washington, DC 📍 Washington, DC 🕒 Shift: 5x8-Hour Days | 8:30 AM – 5:00 PM 📅 Contract Length: 13 Weeks ⏱️ 40 Hours per WeekPosition HighlightsWork within a multidisciplinary hospital care management teamFocus on discharge planning and patient transition of careCollaborate closely with physicians, nursing staff, patients, and familiesConsistent weekday schedule with no rotating shiftsOpportunity to work in a nationally recognized academic medical centerKey ResponsibilitiesProvide clinical social work services to patients and families throughout hospitalizationCoordinate discharge planning and post-acute care servicesSupport patients through the continuum of care from admission through dischargeCollaborate with interdisciplinary teams to ensure safe and efficient patient transitionsAssist with community resource coordination and psychosocial supportAdapt effectively to changing patient care and case management needsRequired QualificationsActive LICSW license requiredActive Washington, DC Social Worker license requiredMinimum 1 year of hospital case management/discharge planning experienceCerner EMR experience requiredStrong communication and care coordination skillsMust reside at least 50 miles from the facilityPreferred BackgroundAcute care hospital experience strongly preferredExperience working in multidisciplinary care teamsKnowledge of community resources and post-discharge planning
