The PT will work closely with physicians, interdisciplinary teams, and families to develop individualized treatment plans that promote independence, restore function, and improve overall quality of life. This role is ideal for a clinician who values autonomy, patient-centered care, and measurable outcomes. This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates HOME HEALTH PHYSICAL THERAPIST | RESPONSIBILITIES: Perform comprehensive physical therapy evaluations based on physician referrals Review patient medical history, diagnoses, precautions, and contraindications Develop measurable goals and individualized treatment plans Provide evidence-based therapy interventions to improve mobility and function Reassess patient progress regularly and modify treatment plans as needed Communicate patient progress and needs to physicians, care teams, and family members Recommend and assist with adaptive equipment to enhance patient outcomes Maintain accurate and timely clinical, evaluation, and billing documentation in compliance with company, state, and federal regulations Supervise Physical Therapist Assistants (PTAs) and therapy aides in accordance with state practice guidelines Participate in staff meetings, training sessions, and continuing education HOME HEALTH PHYSICAL THERAPIST | REQUIREMENTS: Education: High School Diploma or GED (verifiable) Attendance: Commitment to mandatory attendance during the first 90 days. Current Physical Therapist (PT) license required Experience with OASIS assessments preferred Adult therapy experience preferred Strong documentation and communication skills Ability to work independently and collaboratively within a multidisciplinary team Take the Next Step!
