Role Description This is a full-time, on-site role for a Case Management Nurse, located in Martinsville, IN. The Case Management Nurse will be responsible for managing patient care plans, coordinating discharge planning, conducting utilization reviews, and overseeing case management activities. The role involves working closely with patients, families, and the multidisciplinary healthcare team to ensure high-quality care and effective resource utilization. Qualifications Strong foundation in Nursing and Medicine with a registered nurse (RN) license, and relevant clinical experience.Proficient in Case Management, discharge planning, and patient care coordination.Experience in Utilization Management, including the ability to assess care needs and align them with resources effectively.Excellent communication, organizational, and problem-solving skills.Compassionate and empathetic with the ability to support patients and families in emotional and challenging situations.Knowledge of hospice care guidelines and prior experience working in hospice or palliative care is a plus.
