Program Manager, Homeless Outreach

Direct the daily activities of the Outreach team, coordinate with referring community agencies, and provide direct services as needed to individuals accessing services through the Outreach Program. Serves as a member of the organization that welcomes, engages, and serves individuals with complex wellness needs.RequirementsSupervise assigned employees. Recruit, interview, recommend hire, provide training and development as appropriate, provide case consultation, counsel, manage and appraise performance, recommend and provide discipline as needed and as assigned, and recommend termination of employment as necessary.Assist Project Lead in assigning work to program staff and communicate regularly regarding case management of youth and youth outcomes.Support employee retention and professional growth.Build and maintain strong partnerships across CLR departments and throughout the community.Coordinate and manage the day to day operations of the Outreach team. This includes managing relationships with referring agencies, attending any community meetings to coordinate client care across systems, and supervising assigned staff on the team.Ensure that data required by contract is collected and complete. Analyze data in conjunction with Outreach Director.Use data and outcomes to drive practice and quality improvement activities.Collaborate with Outreach Director and CEO to share outcomes and program data with funder/community stakeholders.Function as a member of the CLR management team.Work with the referral sources and community partners to identify SOAR candidates through team meetings, outreaches, and referrals.Initiate paperwork with consumers as referred to program by filing initial documentation of representation with SSA office.Complete interviews with consumers to gather information to complete SSI/SSDI applicationsGather medical records and other information to complete SSI/SSDI applications.Write SOAR Medical Summary Reports for consumer applications.Accompany consumers to appointments at the Social Security Administration.Coordinate visits to medical doctors, psychiatrists, and other specialists to obtain evidence for case.Coordinate case management services with partners and help with providing case management services to consumers when needed.Train community members on the SOAR process.Assist the team with administrative tasks as needed.In addition to the above, other duties as assigned.QualificationsBA in related field preferred. Experience working with chronically mentally ill and/or crisis outreach experience preferred.Experience in managing programs and teams preferred.Strong commitment to the right and ability of each client with a severe mental illness to live in normal community residences, work in market jobs, and have access to helpful, adequate, competent, and continuous supports and services.Strong analytical, interpersonal, written, and verbal communications skills.Demonstrated ability to multi-task, prioritize deadlines, work independently, take initiative, and maintain confidentiality.Valid Washington State driver’s license; proof of insurance.Comprehensive Life Resources is proud to be an Equal Employment Opportunity Employer.
