Well Life Medicine is an integrative physical and mental health clinic with locations in Salem, Hillsboro. We offer a unique integrative medical model with primary care providers, mental health specialists (PMHNPs and therapists), extended hours, urgent care/same day services as well as labs, vaccines. We offer telehealth visits as well as in-person visits.

Job Overview
The Family Physician will be responsible for direct patient care; medical intakes, medication management and follow up appointments. The ideal candidate for this position is patient-oriented, team player and enthusiastic about high quality primary health care. Candidates must be currently licensed/credentialed in Oregon or be willing to do so.
· We offer 30 minute appointments and value relationship based medical care, we really get to know and care about our patients!
Candidates will:
· Perform problem focused visits as well as preventative annual exams, PAPs and well child checks
· Possibility for prenatal care (only if you're interested), but this is not required!
· Perform basic in-office procedures and urgent care services
· Develop patient treatment plans using evidence-based guidelines and protocols
· Cultivate positive therapeutic relationships with our patients
· Utilize patient feedback to monitor and continuously improve patient outcomes
· Simplify their workflow with our easy to use EHR to provide timely and accurate charting and appropriate billing codes for all encounters
· Collaborate with referring physicians and providers to provide timely updates on patient progress throughout treatment
· Collaborate with Medical Director and clinical staff to review treatment plans on a regular basis
Minimal call (we have after hours answering service)
