If you are looking forward to becoming a part of a dynamic team, we would love to meet you!#2025QUALIFICATIONS:Health - Ability to meet performance requirementsEducation, Experience and Skills:Graduate from an approved school of nursing, College or UniversityMust have a current, unencumbered license to practice nursing as a Registered Nurse/Licensed Practical Nurse in FloridaParticipates in continuing education so that he/she has knowledge of current trends and developments in nursing and maintains current licenseMinimum of two years of RAI and Care Plan experience, preferably in a Skilled Nursing Facility (SNF)Must have knowledge of MDS 3.0 including Assessments, CAA's and Care PlansConduct each Assessment and Care Plan with the appropriate participation of health professionalsAbility to organize work procedures, assume responsibility, and tactfully interact with other disciplinesMust be able to communicate well with any member of the Interdisciplinary TeamCollaborate with the IDT to provide guidance on appropriate and precise charting for the medical record PERFORMANCE REQUIREMENTS:Must be able to sit, stand, bend, twist, lift and move intermittently throughout the dayMust be able to lift, push, pull and otherwise move a minimum of 50 poundsAble to see and hear adequately in order to respond and interact with other staff and familiesAble to speak in clear, concise voice in order to communicate with patients, families, and other staff and disciplinesRelate to and work with the elderly, disabled, emotionally upset and hostile ResidentsFrom time to time, you may be required to assume the duties of a floor nurseSPECIFIC DUTIES AND RESPONSIBILITIES:Assign responsibility for completing sections of the MDS to staff who have clinical knowledge about the patientAssure that non-licensed staff who provide direct care to the patients are allowed to contribute information for the assessment processConducts, completes or coordinates other disciplines in completion of the RAI and certifies the completion of the AssessmentSign and certify the CAA Summary FormAttend care planning and enter the care plan decision informationAssist with month-end duties for closeoutPerform other assignments as assigned
