*Assisted Living Registered Nurse **Duties:*- Provide comprehensive case management services to residents- Conduct resident assessments and develop individualized care plans- Collaborate with healthcare team members to coordinate and facilitate resident care- Monitor resident progress and adjust care plans as needed- Utilize utilization management techniques to ensure appropriate use of healthcare resources-Monitors and follows thru with ordering medications and all medication renewals as well as any necessary follow-up with physicians regarding medications.- Coordinate admission and discharge planning and ensure smooth transitions of care- Provide resident education and support to promote optimal health outcomes- Maintain accurate and up-to-date documentation of patient interactions and care plans-Communicates changes in residents and delegates tasks to AL Aides*Skills:*-Possess a sensitivity and aptitude for working with the elderly.-Possess leadership/supervisory skills- Excellent resident assessment skills- Knowledge of medical terminology and procedures- Must be able to prioritize and organize work effectively and efficiently.- Strong communication and interpersonal skills with residents, families and staff If you are a dedicated Registered Nurse with experience in case management, we invite you to join our team.
