Come talk to us about joining the Centerstone team!Job DescriptionSUMMARY OF POSITIONProvides therapeutic interventions to promote personal growth and resiliency in the form of emotional, cognitive, behavioral and/or psychological changes in clients with a primary behavioral health diagnosis. Performs continuing assessment of the client’s needs and functioning, maintenance of appropriate documentation and development, and implementation of personal growth plans consistent with the client’s therapeutic needs.Essential Duties & ResponsibilitiesProvides residential counseling services utilizing evidence-based methodologies to provide excellent clinical services. Prepare for and conduct individual, group, and family therapy for assigned caseload.Assists service recipient in developing an individualized treatment plan that is person centered, family centered, and service centered. Monitor service recipient progress in treatment and participate in treatment plan reviews as assigned. Update treatment plans as appropriate. Submit treatment plans at all stages for medical review, if applicable.Conducts intake for recipient entering services, including explaining services, gathering information needed on intake materials, ensuring service recipient understands all forms, assuring signature on appropriate contracts and consents, and submitting materials for processing, as assigned. Conduct assessment and write assessment summaries, as assigned.Assists service recipients with making linkage with internal and external resources as outlined in treatment plan and in coordination with Integrated Healthcare Services. And maintain appropriate contact with referral sources including providing information on service recipient progress, behavior, and needs (with appropriate releases), and maintaining ongoing consultation when appropriate. Assists service recipient in making discharge plans and provide follow up services as assigned.Attends staff meetings, clinical staffings, and other internal meetings as assigned. Participate on committees as assigned, including quality assurance functions. Maintain appropriate communication with other employees and work as an effective member of treatment team.Monitor urinalysis, breathalyzer, and other testing as assigned.Knowledge, Skills & AbilitiesEffectively communicate via written, verbal, in person and virtual methods.Knowledge of community geographical awareness and of community and agency resourcesSkill in customer service techniquesSkill in problem-solvingQualificationsEducation Level Requires a Master’s Degree in Human Services and demonstrated skill in performing clinical services, or a Bachelor’s Degree in Human Services and 3 years relevant employment experience in positions that allow equivalency and CADC (certified alcohol and drug counselor) is required within two years per SUPR.Years Of ExperienceProviders utilizing electronic health records and/or other automated/electronic documentation systems are required to have computer and keyboarding experience and skills or the ability to develop such skills within required timeframes in order to meet the level of proficiency set by the agency.Certification/Licensure Clinical License preferredPhysical RequirementsStanding – 20%Sitting – 80%Squatting – Occasional Kneeling – Occasional Bending – Occasional Driving – Occasional Lifting – Occasional DISCLAIMERThe duties and responsibilities described in this document are not a comprehensive list and additional tasks may be assigned to the employee from time to time, or the scope of the job may change as necessitated by business demands.Time TypeFull timePay Range$18.20--$25.50BenefitsDelivering care that changes people's lives starts with our employees.
