Crossroad Health Center is a non-profit community health center providing high quality primary health care services to children and adults throughout the tri-state area. Our medical home approach allows patients to develop long term relationships with their medical providers resulting in better overall health. We have been in operation since 1992, serving the under-served in our area. Our mission is to "give honor to Jesus Christ by providing accessible, comprehensive healthcare in partnership with our patients and community leading to healthy, changed lives."*Responsibilities:*· Work independently using his or her best professional judgement while practicing medicine within scope of practice; Work interdependently with other health care providers and health care staff in a team environment· Render care to Crossroad Health Center (CHC) patients in accordance with quality and standards as are established by CHC, the standards of care prevailing in the community, applicable law, and according to state and federal grant requirements for non-discrimination regardless of status, race, sex, religion, national origin, handicap, age, or ability to pay· Provide comprehensive primary care services for adults and children· Provide prenatal and women's health services· Prescribe or administer treatment, therapy, medication, vaccination, and other specialized medical care to treat or prevent illness, disease, or injury· Order, perform, and interpret tests and analyze records, reports, and examination information to diagnose patients' condition· Collect, record, and maintain patient information, such as medical history, reports, and examination results· Monitor patients' conditions and progress and reevaluate treatments as necessary· Explain procedures and discuss test results or prescribed treatments with patients· Advise patients and community members concerning diet, activity, hygiene, and disease prevention.· Refer patients to medical specialists or other practitioners when necessary. Refer patients to other healthcare practitioners or health resources· Collaborate with healthcare professionals to plan or provide treatment· Train residents, medical students, and other health care professionals· Provide on-call services after clinic hours for urgent patient phone calls (Phone coverage only)· Responsible for accurate, timely, and complete documentation in patient records in accordance with CHC policy*Qualifications:*· Excellent written and verbal communication skills· Possesses a high level of personal accountability, ownership, and a rigorous work ethic· Ability to work independently with little to no supervision· Strong time management skills· Ability to function effectively as a team member and facilitate completion of tasks.· Experience in FQHC (Federally Qualified Health Center) industry (preferred)· Requires strong organizational skills, problem solving and analytical skills· Consistently employs sound judgment and discretion in managing patient and employee issues· Graduation from an accredited medical school with a degree of Doctor of Medicine or Doctor of Osteopathy; Must be Board Eligible/Certified· Must possess current, valid unrestricted license to practice medicine in the State of Ohio· Must have current, valid unrestricted DEA registrationJob Type: Full-timePay: $170,000.00 - $200,000.00 per yearBenefits:401(k)Dental insuranceEmployee assistance programFlexible scheduleHealth insuranceLife insurancePaid time offProfessional development assistanceRetirement planVision insuranceWork Location: In person
