The office location is in Idaho Falls, however as the QIDP for REXBURG - You would be expected to be in Rexburg 3 days a week and in Idaho falls 2 days a week.Responsibilities Perform his/her work in accordance with the currently approved method of practice set forth by the State of Idaho, Department of Health and Welfare rules and regulations regarding Program Coordinators (QIDP's), for Residential Habilitation AgenciesShall conduct at least one monthly face to face contact with each of the residential habilitation Participants of Inspire Services.Shall provide Inspire Services written program coordination notes of face-to-face contacts with the Participant within five days of each visitComplete data collection reviews on THERAP each monthTrain staff on THERAP during orientation trainingComplete 6 month and 12-month status reviews within the time limit allowed by the stateWrite behavioral intervention plansShall participate during the Participant's Individual Support Planning process and develop the Habilitation Implementation Plans in a timely mannerShall coordinate the Participants needs with the Plan Developer or the Targeted Service Coordinator as neededAttend ISP's for new and existing participantsTrain staff on participants specific needs and their PIPSWill revise and update the Participant's Habilitation Plan and PIP's as neededMonitor all information of THERAPAssure all documentation for each participant is done correctly, accurately and within correct time limitsCoordinate, oversee and attend doctor appointments and visitsCrisis management / manage behaviorsCoordinate Participant schedulesScheduling of staff and ensure all hours are filled efficientlyWill participate in in-service/training programWill have current CPR/1st AidMaintain high standards of ethical conductAttend a minimum of 8 hours ongoing training annuallyComplete or obtain an age-appropriate functional assessment for participants served within 30 days of initiation of service.Other duties and responsibilities specified by administrator or program managerJob Details Job Type: Full-timePay: $20.00 - $21.00 per hourBenefits Health insurancePaid time offPaid trainingVision insuranceShift Day shiftWork Days Monday to FridayDay Shift (Preferred)Night Shift (Preferred)Overnight Shift (Preferred)Requirements * MUST be able to pass a background check#J-18808-Ljbffr
