See the following link for more information:Our Communities Connected for Kids Program provides services for children and families in the child welfare system, to ensure a community without abuse, neglect and abandonment. Based out of Port St. Lucie, FL, this position identifies post adoptive services to address the needs of substitute and adoptive families and serves as the agency liaison to all post adoptive caregivers. Additional responsibilites of the position include:Conduct one year follow-up contacts for post-adoptive families to identify the type of postadoption services requested or neededDocument outcome of post adoption contacts in FSFN and/or CCWIS.Coordinate with community agencies and partners to promote the development of and access to adoption competent supports and services for adoptive families in Indian River, Martin, Okeechobee and Saint Lucie CountiesRespond to requests from CBC of Origin for post adoption services needs for families adopted elsewhere but residing in CBC service areaParticipate in and provide follow up as needed on child specific staffings related to post adoption families/childrenGather information of the availability and effectiveness of post-adoptive services providedCoordinate monthly adoption support groups and/or newsletterAssist in resolving adoptive parent questions, complaints, grievances, and board payment issues.Evaluate and recommend action on requests to DCF for enhanced maintenance adoption subsidy requestParticipate in development and implementation of the Permanency section of the circuit's five-year Prevention and Permanency PlanComplete Extended Maintenance Adoption Subsidy (EMAS), notification, determination and terminationComplete Annual Cost of living when requested by adoptive parentsComplete the Tuition Waiver for all eligible youth attending college in FloridaCoordinate, manage documentation, and participate in Adoption Review Committee activitiesServe as Point of Contact for the Annual Employee State adoption benefitsParticipate in the quality improvement process and accreditation processPerform other duties as assigned.#sponsoredEDUCATION:Required: Bachelor's degree in human services, social work, or related field from an accredited school or university.Preferred: Master's degree in human services, social work, or related field from an accredited school or universityEXPERIENCE:Required: Three (3) or more years' experience in adoption services and case management. Florida Certification Board Child Welfare case manager and or licensing certification.Preferred: Certification in adoption competence. FSFN and CCWIS experience preferredOther Requirements:Valid FL State Issued Driver's LicenseWork schedule hours will be a core Monday - Friday, dayshift; however flexibility to work outside of regular business hours is required including evenings and weekends due to the needs of the clients.About Communities Connected for KidsCommunities Connected for Kids provides oversight and coordination of the child-welfare system in Indian River, Martin, Okeechobee and St. Lucie counties.
