We are seeking self-motivated, energetic, detail oriented, highly organized, tech-savvy Licensed Social Workers to join our Care Management team. Our Care Managers evaluate our Members needs and enhance their overall wellness through home visits where we do face-to-face assessments. Qualifications: · Care Managers are required to be a Licensed Social Worker (LSW) (MSW) (LISW) or Registered Nurse (RN) with at least one year of paid experience working in home and/ or community-based environments.
