Shift Details : M-F 9am - 5pm, 35hrs/week, Hybrid Job Description: The Transitions of Care Coordinator will fulfill the critical Medicare MOC requirements related to sharing with admitting facilities a member's plan of care as well as informing the PCP of record of their patient's admission and discharge from the acute care setting, skilled nursing facilities and home care. Ensure current plans of care are sent to acute care facilities for admitted members. Ensure PCPs are notified of admissions to acute care facilities and discharge dates.
