Role Description We are seeking a Nurse Practitioner for a full-time, on-site role based in Philadelphia, PA. The Nurse Practitioner will be responsible for assessing patients, diagnosing illnesses, developing care plans, and delivering treatments with a focus on both primary and addiction medicine. Additional duties include disease prevention education, monitoring patient progress, coordinating with multidisciplinary teams, and maintaining accurate patient records to ensure high-quality care. Qualifications Clinical assessment and diagnostic skills, including the ability to evaluate acute and chronic conditions.Experience in family medicine, addiction medicine, or related fields is highly desirable.Strong patient education and counseling abilities focused on disease prevention and health maintenance.Excellent communication and interpersonal skills to work collaboratively with patients, families, and healthcare teams.Proficiency with electronic health records (EHR) and patient record management systems.Valid Nurse Practitioner license and certification for practice in Pennsylvania.Current ACLS/BLS certification.Commitment to providing compassionate, patient-centered care within a diverse community.
