The Family Nurse Practitioner promotes optimum health, prevents illness and injury, and manages acute and chronic health problems for all the plan members.New Clinic Opening June 2026Clinic Hours: M, T, Th: 7:00 am – 3:30 pm, W: 7:00am- 5:00pm, F: 7:00am-1:30pm Clinic Address: 440 Church Street SE, Salem, OR 97312About the RoleThe Family Nurse Practitioner oversees and provides patient-centered, comprehensive, primary care services at the Health & Wellness Center.ResponsibilitiesPerforming episodic, preventive, and/or primary health careDeveloping personalized treatment plans in collaboration with patients and Medical Lead PhysicianConducting physical exams, health-risk assessments, and history reviewsProvide referrals to appropriate medical resource or specialty providers as neededIdentify/respond to emergency situations both within Health & Wellness center and elsewhere on the employer premises, within legal scope of practiceEnsures the use of standardized and consistent medical procedures/processesParticipates in the development of annual Health & Wellness Center goals and objectivesWorks in tandem with Medical Assistant, Patient Advocate, and Services Director to ensure a collaborative, team-oriented environmentParticipate in the development of policies and guidelines (clinical and administrative) to correct and improve service to the client/accountMaintains accurate, timely, and complete Electronic Medical RecordsParticipates in the incident/occurrence reporting process being mindful of Risk Management and mitigation, quality, compliance and client relationshipsEducates and coaches patientsCollaborates with Medical Lead Physician as appropriateResponsible for clinical oversight and health center consultation regarding administration of health care policy and procedures when Medical Lead Physician is not onsiteSupports, leads and models the vision, mission and values of the company QualificationsMaster's degree (MSN)A minimum of 2 years' experience in a Family Practice/Primary Care environmentLicensed as a Registered Nurse and Nurse Practitioner in the state of postingCertification as a Family Nurse Practitioner (preferred)Knowledge of workplace health and safety concepts and OSHA regulationsCurrent Certification in AHA or ARC Basic Life Support for medical providersAppropriate certification to write prescriptions under the authority of the Collaborative Physician in accordance with state and federal guidelinesThorough knowledge and adherence to HIPAA, OSHAA, and clinical quality standardsTraining skills a plusDemonstrated problem-solving and workflow management skillsKnowledge and experience with Electronic Medical Records (we use Athena)Must be willing to develop a thorough understanding of the Mission, Vision, Values, and Patient Promise of Proactive MD and be a champion of them in each patient encounter. Annual salary range DOE: $145,000-155,000 plus Provider Incentive Bonus of up to 5% of annual salary.
