Triage Nurse
Centerpoint Health is a Federally Qualified Health Center (FQHC) seeking a Triage RN or LPN for our community health center. We have locations in Franklin and Middletown, OH. We provide services in primary health care, ob-gyn, and behavioral health. Centerpoint Health has an opening for a part or full time Triage Nurse, LPN or RN with Family Practice experience to join our highly regarded and growing practice. This candidate should embody exceptional interpersonal skills, professionalism and adaptability with a strong desire to work as part of a team.Description of Position/Purpose:The Triage Nurse represents the care team and coordinates patient centered care. Providing nursing care within the relevant legal scope of practice as directed by the physicians and nurse practitioners, the Nurse assists physicians and nurse practitioners with clinical and care coordination aspects of medical care for patients.Essential Duties and Responsibilities:Represent the care team when working with patients. Prepare patients for examination. Obtain patient’s medical history.Verify medication reconciliation. Take and record temperature, pulse, blood pressure and other vital signs to detect deviations from normal and assess condition of patient. Document nursing history and physical assessment for assigned patients. Assess patients for hidden needs. Perform routine and symptom driven tests. Provide initial review of laboratory results. Collect paperwork, forms and specimens from patients. Explain procedures and treatments to patients in a manner that elicits cooperation and understanding. Initiate patient education as prescribed by the physician or nurse practitioner. Administer immunizations. Administer prescribed medications and treatments in accordance with approved nursing techniques. Aid physicians and nurse practitioners during treatment and examination of patients. Prepare equipment for physicians and nurse practitioners. Observe patients, record significant conditions and reactions, notify physicians and nurse practitioners of patients’ condition and reaction to drugs, and significant incidents. Manage and return patient phone calls. Authorize prescriptions as directed. Coordinate ancillary patient care. Complete all paperwork, and update and maintain patient medical records. Respond to life threatening situations based upon nursing standards, policies, procedures and protocol. Participate in department or unit quality improvement activities. Know and adhere to all policies and procedures that pertain to the job position. Collecting pertinent patient information and initiating a decision-making process that categorizes and prioritizes the needs of patient seeking care. This includes telephone and in-person triage. Schedule patient to be seen as needed Give patient instructions per Barton D. Schmitt Pediatric Telephone Protocols 12th Edition. Document findings in EHR.Perform other duties as assigned by the supervisor such telephone encounters and other related items.Certifications/Credentials:Education: Registered Nurse or Licensed Practical NurseCertification and Licensure: Must maintain unrestricted RN or LPN licensure in Ohio.Experience: Prefer 3 years as RN or LPN providing direct patient care in health care center. Primary Care experience preferred.
