Every team member helps design, iterate, and grow.If you’re a nurse who thrives at the top of your license, believes in proactive care, and is ready to help design care delivery from the ground up, this is your moment.About the RoleThe Virtual Nurse is a critical part of our interdisciplinary team, responsible for delivering proactive, high-acuity, person-centered care to patients with complex medical and social needs, all in support of helping people stay safely at home.This role is ideal for a clinically strong nurse who has seen firsthand how gaps in ambulatory care drive unnecessary ER visits and hospitalizations. Whether you come from home health, emergency care, or ambulatory settings, you’ve likely worked with patients who didn’t want to leave home, but they just didn’t have support fast enough to avoid it. This is your chance to change that.What You’ll DoLead patient care coordination and engagement across an assigned panel of patients using a mix of scheduled and as-needed outreach.Conduct initial and ongoing assessments to understand patient needs and create individualized care plans that evolve over time.Support patients through transitions of care, including after hospital discharge, ensuring safe and seamless continuity.Monitor patient status and escalate concerns when clinical changes are noted, collaborating closely with APPs and physicians.Identify patients who require palliative support or hospice and connect them to appropriate resources.Identify barriers to care, such as social determinants of health, and partner with Care Navigators to address them.Drive care plan execution, leveraging Ruby’s technology, resources, and partner networks.Actively participate in team huddles, case reviews, and program development conversations, helping to refine and improve our approach as we scale.Collaborate regularly with the patient’s home health team to execute on the joint care planEnsure accurate and timely documentation within Ruby’s EMR.Serve as a thought partner and leader within the Clinical and Engineering teams, sharing insights and helping shape new workflows and standards.Who You AreAn RN with strong clinical judgment and confidence practicing at the top of your licenseExperienced in home health, emergency medicine, or ambulatory carePassionate about helping patients age in place Skilled in care coordination and remote care deliveryProactive and comfortable with change; you think like a builder, not a followerExcited by new models of care and value-based deliveryComfortable with technology and EMR navigationCollaborative and team-oriented with a flexible, solutions-first mindsetHighly collaborative, valuing teamwork with APPs, MDs, Care Navigators, and external partners.Flexible and proactive, with the ability to manage multiple priorities and patient needs at once.Preferred Qualifications3+ years of experience in home health, ER, care management, or ambulatory complex careBSN preferredExperience with Medicare, Medicare Advantage, or Medicaid populationsFamiliarity with value-based care and transitional care principlesWhy Join Ruby HealthHelp build a new kind of clinical program from day oneWork in a hybrid role with flexibility and purposeJoin a team that respects and amplifies clinical voicesSee your work reduce unnecessary hospitalizations and improve livesOpportunities to grow into future clinical leadership as we scaleOther InformationThis role will primarily be work from home with regular but very modest in person collaboration expectations. For the in-home portion, Ruby requires reliable internet and the ability to participate in video-based patient visits and team meetings.
