As a key member of the healthcare team, you will be responsible for overseeing resident care plans and reviewing medical history for coordination and completion of MDS, care plan meetings, and other related tasks.Responsibilities:Oversee resident care plans and review medical history for coordination and completion of MDSResponsible for conducting care plan meetings and ensuring that all necessary documentation is completed in a timely mannerCollaborate with healthcare professionals to ensure that resident needs are being met and that care plans are being implemented effectivelyDevelop and maintain accurate and comprehensive records of resident care plans and progressParticipate in quality improvement initiatives to ensure that high-quality care is being provided to residentsProvide education and training to healthcare professionals on MDS and care planning principlesRequirements:Must have at least two years of experience completing MDS and be MDS certified (Preferably RAC certified)Must be a Maine licensed RN with Long Term Care experienceBachelor's degree in Nursing or related fieldAbility to work in a fast-paced environment and prioritize tasks effectivelyStrong communication and interpersonal skillsAbility to maintain confidentiality and handle sensitive information with discretionWhat We Offer:$80,000 ANNUALLY competitive salaryOpportunity to join a great team in an ideal, centrally located setting in the Portland areaProfessional growth and development opportunitiesHow to Apply:If you are a motivated and experienced MDS Coordinator looking for a new challenge, please apply to this exciting opportunity.
