Registered Dietician

Full-Time Registered Dietitian Location: Park Ave Extended Care Facility, Long Beach, NY Facility Size: 240 beds Park Ave Extended Care Facility is seeking a dedicated and experienced Registered Dietitian to join our team. Qualifications: Must be a Registered Dietitian with the Commission on Dietetic Registration. Experience in long-term care or short-term rehabilitation is a plus. Knowledge of EMR (SigmaCare) is a plus. Familiarity with Geri Menu is a plus. Key Responsibilities: Conduct new and re-admission assessments. Perform quarterly, annual, and significant change assessments. Monitor and document residents' weights, weight loss, pressure ulcers, and wounds. Oversee residents undergoing hemodialysis and those on enteral feeding. Complete MDS (Minimum Data Set) documentation. Update care plans and participate in care plan meetings. Update meal and snack preferences, enter diets, and assist with menu reviews for residents. Conduct meal rounds and audits as assigned. Provide nutrition education and counseling to residents and their families. Attend morning reports as needed. Maintain strong verbal and written communication with the medical team, including MDs, NPs, nursing, and interdisciplinary teams. Perform related duties as required.
