Compact Licensed Care Management RN

A company is looking for a Registered Nurse - APCM - Compact License Required. Key Responsibilities Proactively monitor patient vitals, symptoms, and clinical data to identify care gaps Develop and manage individualized care plans aligned with physician recommendations Engage with patients to address concerns and provide clinical support through virtual follow-ups Required Qualifications Active Multi-State RN Compact License Associate Degree in Nursing Science 5+ years of recent clinical experience as an RN in an ICU/ER setting preferred Prior experience in chronic care management is a plus EMR experience, preferably in Athena and EPIC
