Job Title: LVN Location: 5 Nursing Home Dr, Claremont, NH 03743 Duration: 13 weeks (Every other weekend/ holiday coverage- required) Shift: Nights Guaranteed Hours: 40 Pay Details: TRAVELER ONLY: For Travelers/Stipend Candidates (If your permanent address is outside the 50 mile radius from the facility): Blended Hourly W2 Pay Rate: $44.00 Blended Weekly Gross Pay Rate: $1,760.00 Minimum Hourly W2 Pay Rate: $18.00 Min. W2 Weekly Gross (Taxable): $720.00 Weekly Stipend (Non-Taxable): $1,040.00 OR For Traveler 1099 Candidates: Pay Rate on 1099 to the Candidate: $44.00 Weekly 1099 Traveler Pay Rate: $1,840.00 Job Requirements: 3+ years of current experience (within the last 6 months) - non-negotiable Must be willing to float to different units IV certification preferred Previous travel experience - highly preferred Experience in rural or small facilities - highly preferred Education: Completion of a 2-year Associate Degree/BSN in Nursing Experience: 3+ years of current experience (within the last 6 months) - non-negotiable Previous travel nursing experience of at least 1 year is required Experience in rural or small facilities - highly preferred • License Required: Must have an active VT LVN license. Required certifications include BLS (AHA) Other Skills required: Monitor residents continuously for changes in mental and physical health; notify the Supervisor of any significant observations and discuss contacting the physician. Receive and provide detailed Resident Status Reports during shift changes. Accurately transcribe and implement physician's orders. Administer medications and treatments as prescribed, documenting in the electronic MAR/TAR system; order and manage unit pharmaceuticals. Supervise LNAs per New Hampshire Board of Nursing requirements; assign tasks based on staff capabilities and provide necessary support. Accompany physicians during resident rounds to address medical needs and facilitate communication. Administer oxygen and feedings as per facility guidelines; perform wound care and understand wound vacs. Conduct regular unit duties, such as checking appointments and monitoring LNA documentation; oversee infection control and unit cleanliness. Admit, discharge, and transfer residents, ensuring proper documentation and notifications. Complete 24-hour reports for assigned units; check new orders and maintain communication with Nurse Manager about resident concerns. Communicate with families to address concerns and inform them of new orders; obtain consent when needed. Handle incoming calls and determine the appropriate response. Manage assigned units during shifts under the direction of the Nursing Supervisor or Director of Nursing Services. Follow regulations for controlled medications, including counting and reporting discrepancies. Assist in orienting new nursing staff; communicate continuing education needs to the DON and facilitate staff training.
