As part of our Village Health Center, you will deliver integrated medical, behavioral health, substance use, and specialty care to individuals experiencing homelessness. You will be supported by a collaborative, interdisciplinary team. You will work closely with our street health team and have direct access to on-site resources, including our new detoxification facility and recuperative care program, allowing you to focus on comprehensive, whole-person care and meaningful patient relationships. The Role As a Physician, you will provide comprehensive primary care to individuals experiencing homelessness, many of whom have complex and unmet medical needs. This is an opportunity to apply your clinical expertise in a setting where your work directly improves patient outcomes and quality of life.Working within an interdisciplinary care model, you will partner closely with behavioral health, case management, and substance use teams to address the full spectrum of patient needs. This integrated approach supports continuity of care, strengthens patient relationships, and enables more effective, whole-person treatment.In addition to direct care, you will contribute to care coordination, quality improvement initiatives, and the ongoing development of services designed specifically for underserved populations - all within a culture grounded in dignity, respect, and clinical excellence. Key Responsibilities Provide comprehensive primary care within scope of licensureDiagnose and manage acute and chronic conditions; prescribe medications as appropriateOrder and interpret labs, imaging, and diagnostic studiesDeliver preventive care and chronic disease managementPerform in-office procedures as credentialedDocument care accurately and timely in the electronic health recordCollaborate with interdisciplinary teams to coordinate care and support patient goalsParticipate in quality improvement and program development initiativesAdhere to clinical, regulatory, and organizational standards Candidate Profile Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited institutionActive California medical license in good standingBoard-eligible or board-certified in Family Medicine or Internal MedicineCurrent DEA certificationCompletion of accredited residency training Key Competencies and Experience Patient-centered, trauma-informed care – Provides compassionate, culturally responsive care to individuals with complex medical and social needs, including underserved populationsClinical judgment and expertise – Applies evidence-based practices to diagnose and manage acute and chronic conditions, often in high-acuity or resource-limited settingsInterdisciplinary collaboration – Works effectively within integrated care teams, partnering with behavioral health, case management, and other providers to coordinate holistic careCommunication skills – Builds trust with patients and communicates clearly across diverse populations and care teamsDocumentation and regulatory compliance – Maintains accurate, timely medical records and adheres to clinical and regulatory standards, including use of electronic health record systemsQuality improvement and adaptability – Contributes to care improvement initiatives and adapts to evolving patient and program needsCommitment to mission-driven care – Demonstrates dedication to serving individuals experiencing homelessness and advancing equitable access to healthcareWork Schedule This is a full-time position. Physicians typically see approximately 14 patients per day, with 30-minute appointment slots to support high-quality, patient-centered care. Work hours may vary based on clinic needs.Compensation and Benefits Pay is based on experience, qualifications, and internal equity.
