The RN works collaboratively with physicians, caregivers, and interdisciplinary teams to develop and implement individualized care plans.Key Responsibilities:Conduct comprehensive patient assessments, including physical, emotional, and environmental needsDevelop, implement, and update individualized care plansAdminister medications, treatments, and wound care as prescribedMonitor patient conditions and document changes accuratelyEducate patients and families on disease management, medications, and home care techniquesCoordinate care with physicians, therapists, and other healthcare professionalsEnsure compliance with healthcare regulations and agency policiesProvide end-of-life care and support when applicableMaintain accurate and timely clinical documentationQualifications:Active Registered Nurse (RN) licenseGraduate of an accredited nursing program (BSN preferred)Current CPR/BLS certificationValid driver's license and reliable transportationPrevious home care or clinical nursing experience preferredSkills & Competencies:Strong clinical and assessment skillsExcellent communication and patient education abilitiesAbility to work independently and manage time effectivelyCompassionate, patient-centered approach to careCritical thinking and problem-solving skillsApply today to be part of a dedicated healthcare team!Prosperity thanks all applicants for their interest in this opportunity.
