Physical Therapist OpportunityMoorings Park is looking for a Physical Therapist to work in our skilled nursing facility. The Physical Therapist is responsible for ensuring and coordinating the quality and safe delivery of inpatient physical therapy services within the Skilled Nursing Facility (SNF), in compliance with all applicable state and federal regulations.The Physical Therapist is responsible for evaluation, treatment planning, and implementation of therapeutic interventions for residents requiring skilled physical therapy services. The therapist collaborates with the interdisciplinary team to promote optimal functional outcomes, safety, and discharge planning.$5000 Sign On BonusContributions:Performs comprehensive physical therapy evaluations, including assessment of the resident's physical, medical, psychosocial, and functional status within the SNF environment.Develops, implements, and updates an individualized Plan of Care in collaboration with the physician, interdisciplinary team, resident, and family/caregiver, based on identified needs and goals.Establishes realistic, measurable, and functional goals, and regularly assesses and documents progress toward these goals.Provides skilled physical therapy interventions including, but not limited to:Therapeutic exercises (active, passive, resistive)Neuromuscular re-educationGait, balance, and transfer trainingFunctional mobility trainingPositioning and postural trainingPain management techniques and therapeutic modalities as orderedMonitors resident response to treatment and modifies interventions as appropriate.Supervises Physical Therapist Assistants (PTAs) through:Instruction and delegation of treatment plansRegular on-site supervision and review of documentationOngoing communication regarding resident status and care progressionTimely documentation of supervisory activities in accordance with regulationsCommunicates resident progress, changes in condition, and recommendations to the physician and interdisciplinary team.Participates in interdisciplinary team meetings, care plan conferences, and discharge planning.Educates residents and families/caregivers on therapy programs, safety precautions, fall prevention, and use of assistive devices.Recommends, fits, and trains residents in the use of adaptive equipment and assistive devices.Coordinates discharge planning, including recommendations for continued therapy services, equipment needs, and safe transition to the next level of care (e.g., home health, outpatient therapy, or long-term care).Completes accurate, timely, and compliant documentation of evaluations, treatments, progress notes, recertifications, and discharge summaries per facility, state, and federal guidelines.Ensures physician orders are obtained, implemented, and accurately documented.Participates in Quality Assurance and Performance Improvement (QAPI) activities and facility in-services.Maintains current knowledge through continuing education and complies with licensure requirements.Provides guidance and direction to ancillary staff and rehabilitation team members.Maintains a professional and cooperative working relationship with residents, families, and interdisciplinary team members.Ensures compliance with HIPAA regulations and maintains resident confidentiality at all times.Assists with orientation and training of new staff as needed.Participates in performance improvement initiatives and supports facility goals for resident and employee satisfaction.Assists leadership with staff training, mentoring, and performance evaluations when requested.Performs other duties as assigned by supervisor.Job Requirements:Excellent oral and written communication skills in the English language requiredDuring emergencies or natural disasters may be required to be on duty for the duration of the disasterGraduate from a physical therapy curriculum approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, or the Council on Medical Education of the American Medical Association and the American Physical Therapy Association.Meet CEU and HIV licensure requirements.Licensed as a Physical Therapist in the state of Florida.Previous outpatient rehab internship experience preferred.Moorings Park Communities, a renowned Life Plan organization includes three unique campuses located in Naples, Florida.
