Advanced Practice Clinician (NP/PA) – ISNP ProgramSchedule: Full‑time, 40 hours/weekLocation: Providence, RI (travel between 5 facilities during shift)Shift: Monday–Friday, 8:00 AM–5:00 PMDress Code: Medical attireDuration: 4–6 months (FMLA coverage)About The RoleThe Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) will support the Senior Community Care (SCC) program, a longitudinal, integrated care model designed to coordinate and deliver clinical services to high‑risk members in facility-based settings. This role is part of an interdisciplinary care team focused on improving outcomes for complex, vulnerable populations.This position offers true work–life balance with no on‑call, no weekends, and no holidays.
