CNA - Care Partner

The primary role of a Care Partner at Cottages of Lake St Louis is to protect, sustain and nurture the up to 10 elders who live in their Cottage. This is done by providing assistance with their activities of daily living and meeting other needs, as they are required. The ultimate goal of the Care Partner is to provide assistance and care which promotes a high quality of life and a positive elderhood. The responsibilities include not just care for the Elders, but also their environment, including cooking, laundry, and some housekeeping.Functions of the job:Recognize/respond to the needs of the elders while encouraging their independenceObserve, monitor, and report changes in the elders' condition to nurses on the Clinical Support TeamAssist elders with personal care functions, including but not limited to:Dental and mouth careBathing, dressing, and groomingToileting and incontinence careAssist in moving, repositioning, and transporting elders into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc. as specific to each elders plan of careAttend and participate in elder care conferencesBe a part of a team of care givers in the home who work collaboratively to provide care in a healthy and safe environmentPrepare and serve meals and nutritional snacksAssist elders with eating and properly document the intake and weights of the eldersProvide care to the elders in a gentle, empathetic, efficient, and timely mannerLaunder clothing, linen, and other articles per guidelinsRoutinely clean and maintain the environment of the homeRelay grievances or suggestions to the GuideEach Care Partner works with the other Care Partners in their Cottage as part of a Self Managed Work Team (SMWT). The SMWT works collaboratively with the nurses who are part of the Clinical Support Team to direct all matters related to skilled nursing services. For all other matters the Care Partner and SMWT report directly to the Guide of their Cottage.RequirementsRequirements:Certified Nurse Assistant certificationHigh school graduate or equivalentTwist, stoop, bend, squat, lift, and carry items up to 25 poundsReach above, at, or below shoulder heightWalk or stand a majority of the timeCommunicate orally and/or in writing with elders and coworkersAbility to visually monitor elders from a distance of up to 35 feet, and to hear elder's calls, coughs, and alarms
