This will be a clinical intake coordinator for home health processing and scheduling referrals. Coordinates incoming calls from patients, family, physicians, and disciplines. Ensures that all personnel are informed of patient's hospitalization and follow-up with patients admitted to other hospitals. Coordinates small program(s) with limited budget/impact. Applies substantial knowledge and experience to perform a wide range of advanced activities and/or determines how to use resources to meet schedules and goals; serves as working supervisor for team or work group.
