We are seeking a caring and dependable Certified Nursing Assistant (CNA) to join our interdisciplinary team and provide personal, hands-on support to hospice patients in their homes.What You’ll DoAs a hospice CNA, you’ll play a vital role in helping patients and families through meaningful support, including:Providing personal care based on the individualized care plan (bathing, grooming, feeding, dressing, toileting, etc.)Maintaining patient comfort and safety in their home environmentMonitoring and reporting changes in the patient's condition to the RN Case ManagerAssisting with transfers, mobility, and range-of-motion exercisesEducating and supporting families on patient care techniquesDocumenting clinical notes accurately and on timeParticipating in team meetings and quality improvement initiativesQualificationsActive CNA certification in the state of employmentGraduate of an approved Certified Nursing Assistant programMinimum 1 year of nursing assistant experience (hospice, home health, or community-based preferred)Current BLS/CPR certificationValid driver’s license, reliable transportation, and car insuranceAbility to work independently and compassionately in home settingsExcellent communication, professionalism, and time management skillsPlease note: Legacy Hospice is a drug-free workplace.
