Responsibilities:Responsible for supporting Medical Social Workers (MSW) team in direct patient work, administrative work and general dutiesManage routine cases assigned by MSWs independentlyProvide training for new social work and administrative assistantsConduct financial assessment on eligibility of applicants for the various financial schemesConduct field visits and facilitate discharges by accompanying patients home or to social service organizationsAble to independently take on inpatient and outpatient casework, doing follow up work for discharged patients and networking with community services to facilitate seamless careRequirements:Min Diploma in Social Work / Psychology / Counselling or related fieldCertificate in basic counselling skills or related skills will be added advantageJ-18808-Ljbffr
