This role focuses on assessing, treating, and managing wounds while ensuring high-quality patient care, compliance, and strong care coordination.Key ResponsibilitiesPerform comprehensive wound assessments and develop treatment plansProvide weekly patient visits (or as clinically indicated)Prescribe treatments and manage wound-related orders within scopeDocument all patient care accurately and timely (same-day)Coordinate care with agencies, physicians, and internal teamsEscalate care when clinically appropriateRequirementsActive NP or PA license (unrestricted)Strong clinical judgment and communication skillsAbility to work independently and collaborativelyWillingness to travel locally to patient homes/facilitiesSchedule & TravelFull-Time: MondayFriday, 8:00 AM 5:00 PMPart-Time / Per Diem: Flexible based on patient volumeTravel required (up to ~55 miles)No on-call requiredOur GoalDeliver safe, compliant, and effective wound care that improves patient outcomes and supports healing.
