This role provides direct patient care in the home, skilled nursing facilities, and other care settings, ensuring patients and families receive high-quality, individualized support during end-of-life care.What We OfferMedical, dental, and vision insurancePet insurance with HSA optionsSupplemental and life insurancePaid time off (PTO) equivalent to 3 weeks annually6 paid holidays401(k) with company matchNight shift stipend: $300 per shiftMileage reimbursementCompany-branded scrubs providedMonthly employee celebrations recognizing achievements and milestonesA collaborative culture built on open communication, teamwork, and continual learningHourly compensation range: $36 - $42.50/hour for patient visits, based on experience, direct hospice/home health experience, and specialized clinical expertise.Primary ResponsibilitiesProvide skilled nursing care to hospice patients in accordance with physician orders, hospice policies, and regulatory standardsAdminister medications, monitor patient conditions, and document care accuratelyEducate patients and families on care plans, symptom management, and comfort measuresCollaborate with interdisciplinary hospice team members, including RNs, social workers, and hospice aides, to support patient care goalsRespond promptly to patient and family needs, including symptom management and urgent care situationsMaintain compliance with state regulations, company policies, and Medicare guidelinesParticipate in weekend on-call rotationRequirementsCurrent Licensed Practical Nurse (LPN) license in the state of WashingtonActive BLS certificationPrevious hospice, home health, or palliative care experience strongly preferredStrong clinical assessment and critical thinking skillsExcellent communication and interpersonal skillsAbility to work independently and manage evening patient caseloadsValid driver’s license and reliable transportationSalary Description$36 - $42.50/hour + $300 night shift stipend
