DescriptionSummary:Apply fast, check the full description by scrolling below to find out the full requirements for this role.The Emergency Department Navigator helps the patient and/or caregiver navigate the complex healthcare system by complimenting the services delivered through the Emergency Department. The Emergency Department Navigator does not provide clinical care and does not extend or substitute for the more specialized services of a doctor, nurse, or social worker.Responsibilities:Meets expectations of the applicable OneCHRISTUS Competencies: Leader of Self, Leader of Others, or Leader of Leaders.Bridge cultural barriers between communities and the healthcare systemProvide culturally appropriate and accessible health education and informationAssure that people get the services they needProvide informal counseling and social supportAdvocate for individuals and communities within the health and social service systemIdentify barriers and circumstances that led the patient to the emergency departmentNavigate the complex healthcare system by:Providing patient with information and linkage to a primary care provider if patient does not have a providerScheduling follow-up appointment with provider that patient will keepProviding patients and family caregivers with information and linkages to health and social support services including discounted prescription medications and transportationProviding patients with complex needs with direct referrals to the Community-based Community Health Workers for continued follow-up and assistance in the communityContacting patient within 48 hours of discharge as follow-upUnderstand about medical homes and the proper use of primary care and the Emergency Department.Develop Navigation and Accountability PlanExcellent customer friendliness and communicationKnow available Community Resources and how to refer appropriately to identify needed services.Perform all other duties as assigned. xhyhwjd Requirements:Education/SkillsHigh School Diploma or equivalent requiredAssociate or Bachelor's Degree preferredBilingual (Spanish) preferredExperienceHealthcare background preferred.Licenses, Registrations, or CertificationsBLS preferredWork Schedule:PRNWork Type:Per Diem As Needed
