Reviews prescriptions for possible therapeutic problems,contraindications, interactions, allergies, and appropriate dosing. Discusses prescription clarifications with the prescriber or their office when necessary. Documents actions appropriately. Provides patient counseling regarding drug therapy. Provides drug information to health care professionals. Verifies the data entry and filling of all medications entered and prepared by pharmacy technicians. Follows all federal and state laws and regulations and hospital policies and procedures.Minimum RequirementsBS Pharmacy, MS Pharmacy, or Pharm.
